  • Photocopy of Report Card Football
  • Football participants need a recent wallet size photo

Waiver and Release of All Claims and Assumption of Risk:

I affirm that the participant ('Player') has my consent to participate in any program(s) offered by The Clovis Clash Youth Football and Cheer INC.  I recognize and acknowledge that there are certain risks of physical injury to participants in these programs, and I agree to assume the full risk of any and all injuries, damages or loss, regardless of severity, that my minor child or I may sustain as a result of said participation. I understand that there is an element of risk inherent in youth sports, and as parent/legal guardian of the Player, I take full responsibility for his/her actions and physical condition. Furthermore, I understand that certain risks, dangers, and injuries due to things such as inclement weather, slipping, falling, poor skill level or conditioning, carelessness, horseplay, unsportsmanlike conduct, premises defect, inadequate or defective equipment, inadequate supervision, instruction or officiating, and all other circumstances inherent to youth sports programs exist. Therefore, it is recognized that it is impossible for The Clovis Clash Youth Football and Cheer INC to guarantee absolute safety.  I release and agree to indemnify and hold harmless The Clovis Clash Youth Football and Cheer INC and its representatives, board, volunteers, coaches agents, and any county, town, school district, or venue in which program is being held, from all claims or liability for loss, damages and/or injuries, to the named Player and/or any third parties’ person or property; caused by and resulting from any causes whatsoever.


It is important that there is an understanding between parents and The Clovis Clash Youth Football and Cheer INC regarding the no refund policy. In order to run this league, The Clovis Clash Youth Football and Cheer INC has to prepay for insurance, field & restrooms usage, referees, refurbishment of the football gear, new gear and other required administrative expenses. We prepay for most things prior to the season starting and can't recoup them in the event your child decides he/she doesn't want to play or it is out of our Control such as Coivd-19. It is for that reason The Clovis Clash Youth Football and Cheer INC has a strict refund policy: At the time of registration, participants can purchase Regsaver Registration Insurance through their direct website at registrationsaver.com.  This is a 3rd party insurance vendor and the Clovis Clash Youth Football and Cheer INC is not affiliated with Regsaver and any claim for a refund through this insurance policy shall be made directly through Regsaver @ registrationsaver.com If you have any questions or concerns please email at support@theclovisclash.com or go to the contact us page. We appreciate the parents and without you there is no The Clovis Clash Youth Football and Cheer INC.
