YOU MUST HAVE A PHYSICAL ON FILE TO DO SUMMER VOLLEYBALL. If you are new to Tribe, or have not sent us your physical within the last two years please download the (1) Physical Evaluation Form and (2) Verification of Enrollment Form. Complete both and return them, along with a copy of the athlete's birth certificate to registration@tribeathletics.org. If you registered for a Tribe sport for the 2022-2023 season, these documents should be on file.
