IMPORTANT: READ BEFORE SIGNING (PLAYER’S WAIVER AND RELEASE)

I, as the parent/guardian of the player being registered herein (the “Player”), hereby gives White City Futbol Club Inc. (“WCFC”) consent to take and use images of the Player, including any motion picture or still photographs captured during the Player’s participation in WCFC ‘s activities, for any purpose in connection with WCFC’s activities, including the use of such images on WCFC’s website and use in WCFC’s advertising, promotion and marketing activities (collectively referred to as the “Purposes”). I, as parent/guardian of the Player, consent to the Players name being displayed in connection with the use of such images. I am aware that by signing this consent I am giving permission to WCFC to use such images for the Purposes.

In consideration of the acceptance of this registration form White City Futbol Club (WCFC), I, on behalf of the Player named above, for the Player, his / her heirs, executors, administrators, and assigns, waive any claim(s) to which he / she may become entitled for injury or damages and release the WCFC Board and all other organizers, sponsors, representatives, their agents and employees and or any other person or organization assisting with providing WCFC’s soccer programs or related activities from any claim(s) for compensation or damages resulting from any injury suffered by the Player as a result of participation in or traveling to/from scheduled events.
