ROWAN'S LAW
Dear player and/or parent/guardian of a player and Coach,
Rowan’s Law requires all players under 26 years of age and all parents/guardians of players under 18 years of age and coaches to review Ontario’s Concussion Awareness Resources and OBA’s Concussion Policy and Code of Conduct on a yearly basis. Any member who has not met the requirements of Rowan’s Law will not be registered.
You are required by law to review Ontario’s Concussion Awareness Resources, which can be found here (https://basketball.on.ca/coaching/concussion-resources/).

I acknowledge that I have reviewed Ontario’s Concussion Awareness Resources and OBA’s Concussion Policy and Code of Conduct, as required by Rowan’s Law.

Release of Liability, Waiver of Claims and Assumption of Risks and Indemnity Agreement
Please read carefully
I agree to abide by the rules and regulations of the West Ottawa Basketball Association and agree to use the facilities and equipment in a manner consistent with its intended use and application. I further agree to wear appropriate safety equipment while participating in the program/activity.
The program/activity is considered a “High-Risk Sport’ and this physical activity is designed predominantly for healthy individuals. If you have been inactive, have health or medical concerns, you are advised to consult with your physician prior to participating in the program. Participation in this sport and physical activity involves various risks, dangers and hazards, including but not limited to:

Minor or major bone and spinal fractures
Rope, wire, netting burns
Scrapes, abrasions, lacerations
Head or body bumps and bruises
Muscle, tendon, or ligament strains or sprains
Head concussion
Caused by, but not limited to:
Overstraining or exceeding physical limitations
Slips, falls and other gravity-related mishaps
Impact against a hard surface
Human error
Disregard of guidelines, rules and standard practice
Accelerated speeds and projection
Collision with others
Inattention or ignorance
Rough equipment conditions, splinters, chalk dust, etc.
Equipment failure and design
Geographic and land formation hazards
Environmental hazards
Pollution hazards
Animal actions or behaviour

An individual's participation will be deemed to indicate acceptance of such risks and injury. Therefore, the West Ottawa Basketball Association accepts no responsibility for such activity-related risks. West Ottawa Basketball Association representatives do not have medical training; if you have health or medical concerns, talk to your doctor.
I understand and agree that my participation in the program/activity is not a requirement of the West Ottawa Basketball Association. I further agree that in the event of personal injury or property loss, as a result of my participation in the program/activity, I hereby release, waive and discharge the West Ottawa Basketball Association from all liability to me, my heirs, executors and administrators. I accept full responsibility for my actions and obligations and I will not hold the West Ottawa Basketball Association, its employees, volunteers, agents and instructors liable.
I am aware of the nature and effect of the Release of Liability, Waiver of Claims and Assumption of Risks and Indemnity Agreement. I am executing this release and waiver of liability agreement freely and without any compulsion on the part of the West Ottawa Basketball Association, I acknowledge to having read this entire agreement.
By accepting the terms of this document, I am freely assuming all risks of injury and other risks associated with participating in the program/activity. I acknowledge having read, understood and agreed to the conditions contained in this Agreement.

Privacy Policy for West Ottawa Basketball Association
This Privacy Policy describes our policies and procedures on the collection, use and disclosure of your information when you use the service and tells you about your privacy rights and how the law protects you. The company may at times take and post video and/or pictures of our volunteers, referees, players, coaches and teams throughout the season. I hereby grant permission to West Ottawa Basketball Association to use photographs and/or video of me taken in publications, news releases, online, and in other communications related to West Ottawa Basketball Association.
