Consent to Post or Publish Player Information
Hockey Calgary would like to share information and communicate with parents by
highlighting Associations, Players and Player activities in a variety of public forums for
non-profit purposes. The following are examples only and are not meant to be an all-
inclusive list of how Player personal information and activities may be shown: displays
during HC sponsored events; development sessions; other HC related activities, HC
sites; HC publications; or posting or publishing on Hockey Calgary’s web site. Players’
photographs and activities that are showcased may be identified using the Player's first
name, last name or initial and Association only. In order to comply with Alberta's
Personal Information Protection Act (PIPA) and the Copyright Act, Hockey Calgary is
requesting consent from parents to post or publish Player information and activities on
various public forums.
Please note the following information:
• Once photographs, Player names and other identifying information or Player activities
are released in any public forum, Hockey Calgary cannot control or prevent the further
distribution or use of the material by those who access the information.
• Parents are under no obligation to consent; it is their voluntary decision to do so. If
you do not agree to this document, this indicates a refusal to consent.
• Your consent will be valid for the duration of the Hockey Season. You may withdraw
your consent prior to posting or publication by notifying the Executive Director in
writing. You may at any time instruct us to remove the photographs or activities from
Hockey Calgary sites such as Hockey Calgary displays or from Hockey Calgary’s web
site by notifying the Executive Director in writing.

Consent for Release
I, being the parent/legal guardian of the Player named in this registration, have read
and understand the information provided in this agreement. I voluntarily give Hockey
Calgary consent to post or publish my or my player's photographs, first name and last
name or initial, Association and activities in various public forums as described above
for non-profit purposes. I understand my consent is valid for the current Hockey Season
unless I revoke my consent prior to posting or publication by notifying the Executive
Director in writing.
Consent to Receive Communication from Hockey Calgary
Hockey Calgary would like to share information and communicate directly with parents
from time to time. In order to comply with Alberta's Personal Information Protection
Act (PIPA) and the Copyright Act, Hockey Calgary is requesting consent from parents to
receive direct communication from Hockey Calgary.

Please note the following information:
• Parents are under no obligation to consent; it is their voluntary decision to do so. If
you do not agree to this document, this indicates a refusal to consent.
• Your consent will be valid for the duration of the Hockey Season. You may withdraw
your consent prior to posting or publication by notifying the Executive Director in
writing.
Consent for Communication
I, being the parent/legal guardian of the Player named in this registration, have read
and understand the information provided in this agreement. I voluntarily allow Hockey
Calgary to provide communication from time to time to me directly. I understand my
consent is valid for the current Hockey Season unless I revoke my consent prior to
posting or publication by notifying the Executive Director in writing.
From time to time the Calgary Sports and Entertainment Corporation would like to communicate
information directly to Hockey Calgary Members on things such as Minor Hockey fundraising, special
experiences, prize draws, and free ticket offers.
I do hereby give consent and I certify that I am the Parent or Legal Guardian of the registered child(ren).
I hereby give / do not give the Wolverines Hockey Club the right and permission to publish, without compensation; photographs, video recordings or audio recordings taken of my registered child during hockey related activities and events during the 2026 - 2027 hockey season. This media may be used in whole or in part by Wolverines Hockey, and may be used for promotion, illustration, advertising, or publication on the Wolverines website. All recorded media, prints and created media from the content shall constitute the property of the Wolverines Hockey Club.

We will register you now and send you an email closer to the start of the season when payment is is required.

REFUND & WITHDRAWAL POLICY: The refund policy is outlined in the Standing Policies and Procedures document which is available on our website. Refunds for hockey registration are available from WHC when a player withdraws for the season.  No refunds will be issued for missed games or portions of the season. 

Refunds will be issued as follows:

  • Prior to the player’s first assigned evaluation ice time (or conditioning skate if applicable) – 100% minus a $100 Admin fee
  • Prior to being notified of team placement – 50% minus a $100 Admin fee
  • No refunds will be provided after teams have been formed except for withdrawal due to medical reason or family relocation out of zone 9

In such cases, refunds will be pro-rated. Medical withdrawal requirements are outlined below:

  • Medical certificate (on medical practice letterhead)
  • Date of diagnosis
  • Medical reason/recommendation for non-participation
  • Printed name and signature of medical professional
  • The refund will be prorated based on the date of diagnosis

Once a medical refund has been given, the player will be removed from the official roster and will no longer be allowed to participate in team activities.  The roster spot will now be considered vacant and could be filled by a new player registration.

No refunds will be issued for suspended players.

Notice of withdrawals will only be accepted via email to the Manager of Operations at info@wolverineshockey.ca. The date and time stamp of the email will be used to determine eligibility and what refund scenario is applicable.
