Terms & Conditions/Waiver & Release of Liability
In consideration of the organizers allowing my child to participate in team activities in the Wyoming Ball Hockey league, I agree to the following:
•I am aware of the possible risks, dangers, and hazards associated with the player’s participation in team activities, including the possible risk of severe or fatal injury;
•To assume all the risks arising out of, associated with or related to the player’s participation in these activities;
•To be solely responsible for any injury, loss, or damage that may have been caused by the organizers. The Town of Plympton-Wyoming Parks Board, Town of Plympton-Wyoming and/or Wyoming Ball Hockey League are in no way responsible for any injuries;
•To release the organizers of responsibility for any claims, demands, actions, and costs that may arise out of participation in any team-related activities, even though such claims, demands, actions, and costs may have been caused by the negligence of the organizers;
•I grant permission to the organization to photograph and/or record the player’s image and/or voice and to use this material to promotethe organization through the media of newsletters, websites, print and/or other form. I understand I waive any claim to remunerationfor use of materials used for these purposes.
I acknowledge that I have read this agreement and that this agreement is binding upon myself, my heirs, executors, administrators and representatives.
